New Patient Forms for TeleMedicine Clinic
Our clinic will be using TeleMedicine to continue care for our patients to follow the current guidelines by the WHO and CDC with regard to social distancing.
Before your appointment, we will need a copy of the following:
1. Driver’s License
2. Insurance Card – Front & Back
3. New Patient Forms (click here for forms)

If you do not have a scanner, you can take a picture on your phone and upload it via the link.

TeleMedicine Clinic Instructions
Our clinic will be using TeleMedicine to continue care for our patients to follow the current guidelines by the WHO and CDC with regard to social distancing.
Any desktop, laptop, phone, or tablet can be used for your appointment. We will be using the ZOOM.US platform for your appointment. It is preferable that you have a camera, microphone, and speaker attached to your device. A camera is not necessary, but a microphone and speaker are required.
At your scheduled appointment day and time, please follow these instructions:
1. Open your web browser (chrome, firefox, safari, explorer)
2. Type in the web address: https://zoom.us/j/9165732595
3. A box will open to download or open the Zoom application.
• Most phones/tablets require that you download the zoom application
• If you do not wish to download or use the Zoom app, use your laptop/desktop computer to connect and click on the link “start from your browser”.
4. Once you have opened the zoom app, chose “Join with Video”. Your computer may ask for permission to use your video camera and or microphone. Please accept
5. You will then be in Dr. Panero’s “virtual waiting room”
6. When admitted into room, choose “Join with Audio”. Your computer may ask you for permission to use its microphone, please accept.
7. Please wait, we will be seeing patients one at a time. Once Dr. Panero is ready for your telemedicine appointment, he will start the video session.
